1. The validity period of the Affidea Premium service package is 12 months from the date of signing this document.

2. The product will provide access to the following services at a preferential price:

• Free access to privately charged diagnostic tests for one year

• Coverage of the National Organization for the Provision of Health Services (EOPYY) participation for all examinations (including the €1 or €3 contribution)

• Free access to medical visits at Affidea Polyclinics

• Priority in scheduling appointments for ultrasound, triplex, and all imaging examinations

• VIP telephone line for immediate and personalized service

• 20% discount on molecular tests and 10% discount on anesthesiologist fees

3. The benefits apply to all Affidea-owned centers nationwide.

4. The Affidea Premium product cannot be canceled and may not be used by any person other than the Customer.

5. No doctor’s referral is required for the examinations included in the Affidea Premium package, except for radiation-related examinations, for which a medical referral must be provided.

6. The benefits of the Affidea Premium package are non-transferable, cannot be exchanged for cash, and cannot be offset against other services.

8. The use of the services (excluding blood tests) requires prior appointment scheduling.
